Remote Care Manager
Delivers telephonic health coaching and chronic disease management services to patients through digital health platforms, focusing on care plan development, medication adherence, and health outcome monitoring
Key Responsibilities:
- Provide remote patient coaching and care coordination to improve health outcomes for chronically ill patients
- Develop comprehensive care plans in collaboration with patients, families, caregivers, and healthcare providers
- Monitor patient outcomes, treatment adherence, and care plan effectiveness through telephonic and digital touchpoints
- Coach and educate patients on chronic condition management, preventive care, and physician directives
- Facilitate enrollment in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration programs
- Conduct bilingual telephone outreach to English and Spanish-speaking patient populations
- Maintain accurate documentation and communicate updates to primary care providers regarding patient progress
Skills & Tools:
- Medical background (Medical Assistant, Certified Nurse Assistant, or pharmacy training)
- Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) program experience
- Telehealth platforms and remote monitoring technologies
- Electronic health record (EHR) systems and documentation tools
- Microsoft Office Suite and healthcare software applications
- Bilingual communication skills (English/Spanish)
- Patient education and health coaching methodologies
- Care coordination and case management protocols
- Strong customer service and interpersonal abilities
